Returning Student Registration
1. COMPLETE and SUBMIT the following online forms on or before TUESDAY, APRIL 30, 2019.
2. DOWNLOAD and RETURN the Signature Acknowledgement to the main office on or before TUESDAY, APRIL 30, 2019.
The Registration fee of $1,045.00 will be added to your SmartTuition account in May 2019, due on your predetermined payment date (no later than May 31, 2019).
3. RE-ENROLL with SMART TUITION
Parents are asked to visit www.enrollwithsmart.com and complete the re-enrollment for the 2019–2020 school year by May 31, 2019.
Choose option: “I HAVE a Smart Tuition Account.”
4. All tuition and fees MUST BE CURRENT to register for the 2019–2020 school year.
The receipt of registration materials and the payment of the fees does not constitute automatic invitation to St. Lucy's.

In return for my daughter’s educational program at St. Lucy’s, I make this contract to provide for the financing of this educational program. The contributed services of the faculty and staff, the projects of the Parent Club, the Parent Volunteer Program, the Boutique and BASH events, plus any donations or pledges in the Growing for You Fund (see form 5), will supplement the tuition to provide for the total cost of the educational program.
The fee covers the Technology/iPad Program, activities, curriculum, insurance, testing, ASB cards, lockers, library, school newspaper, etc.
Tuition for 2019-2020 will be billed through Smart Tuition.
ST. LUCY’S SINGLE STUDENT RATE

Family rates are extended to the oldest sibling living within a single household attending St. Lucy’s and/or Damien High School.
The Damien student will pay the Damien tuition family rate. The St. Lucy’s student will pay the rates described below:

PARENT VOLUNTEER PROGRAM
The Parent Volunteer Program is part of the financial obligation with the school. Parents are required to complete 20 hours of service per family, per year. Parents must be willing to donate funds in lieu of time if they are not able to help with school projects. The rate of $25 per hour or $500 for 20 hours will be charged.
POLICY CONCERNING REFUNDS
If a student withdraws from St. Lucy’s, prepaid tuition will be refunded for the quarters not yet started. Registration and other fees are non-refundable.
We, the undersigned parent/guardian(s), request admission for the above named student into St. Lucy’s Priory High School for the school term beginning in August 2019, and ending with graduation, withdrawal or dismissal. We agree to the conditions governing admission, continued attendance, and cooperation for the duration of her enrollment in St. Lucy’s.
SUSPENSION AND/OR DISMISSAL
St. Lucy’s depends upon the support and good will of all parents and guardians in order to educate and care for all students. This form confirms your support and cooperation with the school’s policies and its mission to the academic and spiritual formation of the young women attending St. Lucy’s Priory High School.
As a parent/guardian, I understand that the school does not assume responsibility for paymentof a physician in any case. In an emergency that necessitates immediate hospital care, paramedictransport is dictated by the physical condition and closest hospital available.

REGARDING STUDENT DRIVERS AND CARPOOLS IN EMERGENCY SITUATIONS:
St. Lucy’s students represent all members of the school community: administration, faculty, staff, the current student body, families, and all of St. Lucy’s alumnae and friends. The school and team sport uniforms students wear identify them as members of the St. Lucy’s community. Uniforms are worn daily unless specified by the administration. School uniforms must be purchased through Dennis Uniform Company. Wearing uniforms properly and with pride reflects positively on students and all of the St. Lucy’s community.
St. Lucy’s Priory High School seeks to maintain an atmosphere that is conducive to the highest quality of academic pursuit and conduct. Since the appearance of students contributes greatly to an attitude in school and the respect they earn in public, all students are expected to adhere to the following sections regarding uniform expectations.
Parents are responsible to see that their daughter leaves home properly dressed and groomed for school and school-sponsored activities. Parents are to ensure that their daughter regularly follows conventional standards for personal hygiene. All members of the school community are expected to support compliance with the dress and grooming policy.
In all instances, the school reserves the right to regulate against unbecoming fads or fashions that reflect negatively on the person and/or school.
It is the right and responsibility of the faculty to enforce the dress code.
Parents are informed that students may serve detention for choosing not to follow school policies. Parents may be called to bring proper clothing or students may be sent home if they do not comply with school policy.
GROOMING AND CONDITION OF UNIFORM
Students are required to look neat and attractive at all times. Anything that could be considered excessive by way of skirt length, make-up, hairstyles, hair length, hair color, inappropriate or gaudy jewelry, visible tattoos, fingernails, or lack of refinement in dress is indicative of poor taste for school wear. Students may not wear curlers, rollers, head coverings, hats, scarves, forehead bands, etc. to school at any time.
Uniforms and shoes must be clean, unmarked and in good repair at all times. If for any reason a student is unable to be in uniform, a signed note from her parent is REQUIRED. The note is presented to the Principal or Assistant Principal BEFORE school and a permit slip is issued.
ACCESSORIES: Jewelry may not be worn in nose or parts of the face or body other than in the ears. Body-piercing jewelry and plugs are not permitted.
BODY PAINT/TATTOOS: Students may not wear body paint or tattoos to school or at any school activity.
HAIR COLOR/STYLES: Extreme hair colors such as red, pink, green, etc. are not acceptable. Bleaching, unnatural streaking of hair or extreme hair-styles are not permitted. Multi-colored or two-toned hair is not permitted.
St. Lucy’s Priory High School offers a competitive tuition and parent support is crucial to maintaining a reasonable rate of tuition for all families. Below is a list of annual parent requirements.
Every family is required to volunteer a minimum of 20 hours. A complete list of opportunities to fulfill service hours will be provided to all incoming families. “Home for the Holidays”
The Annual Christmas Boutique is a fundraiser in support of St. Lucy’s Priory High School. All families are required to do one of the following: Submit a $50 monetary donation or make a donation of merchandise valued at $50 or more. Additional information will be provided regarding deadlines for donations.
BASH (Building a Scholastic Heritage)
The Annual BASH is the largest annual fundraiser for the School. All families are required to do one of the following: purchase two tickets to attend the event, make a $150 monetary contribution or submit an item valued at $150 or more. Additional information will be provided regarding deadlines for contributions.
Every student at St. Lucy’s is required to sell one book of 10 raffle tickets valued at $100. A drawing for the raffle is held on the evening of BASH.
Growing For You Fund
The Growing For You Fund is the parent component of the Annual Giving Campaign. All families are expected to participate in order to bridge the gap between tuition and the actual cost of educating every student. The money raised in the Growing For You Fund keeps tuition at a reasonable rate for all St. Lucy’s families. Your participation provides support for short and long term goals at St. Lucy’s. All families will receive additional information about how you can make a difference in the Growing For You Fund.

St. Lucy’s, Damien and Pomona Catholic offer three (3) different daily bus transportation options to and from school.
A digital copy of the 2019-2020 signature sheet will be delivered to you via email upon completion of the 2019-2020 Online Registration Forms. Please be sure to provide a valid and accurate email address within the final step of this online registration registration form.
This signature sheet must be returned to the St. Lucy’s main office with original signatures (email, scans or faxes will not be accepted) by TUESDAY, APRIL 30, 2019
